Bridging the Education and Practice Chasm(s)

• Describe the apparent disconnects in health professions’ education that challenge the smooth entry into practice by new graduates
• Identify the rationale for the establishment of centers of innovation and/or transformation by national organizations
• Describe the salient outcomes from a June 2-3 “Bridging Pharmacy Education and Practice Summit” coordinated by AACP and six planning partner organizations

IPE Meets CP: Bridging the Gaps Problem-Solving Rounds with Practitioners
Practitioners from different health professions will join participants to discuss interprofessional education for collaboratice practice (IPECP) from both the education and practice sides, with the goal of determining strategies to bridge the gaps.
Learning Objectives:
• Develop a more comprehensive understanding of collaboration in practice settings
• Describe obstacles in translating IPE to CP
• Identify ways to improve IPE for CP
